Summer Camp Field Trip Permission Slip
I give my child/children permission to be transported for fields trips during Summer Camp with staff of Let's Create, LLC.

Occasionally, we take photographs and videos of your children participating in various creative arts activities to post around the studio space and may send some home with you. Please check this box i if you give permission for your child’s photo to be taken for classroom use.

We have Facebook and Instagram pages which will be used to share news, updates, and information about the program. We also have a website to promote the program. All sites will include photographs of children participating in activities. Please indicate below if you give permission for your child’s photo to be included on the online sites.
Sunscreen and Bug Spray Application Permission
YES, I give permission to the staff at Let’s Create, LLC to apply the sunscreen and/or bug spray that I have provided for my child(ren) to my child(ren), as needed,
I give permission to the staff at Let’s Create, LLC to apply the sunscreen and/or bug spray that I have provided for my child(ren) to my child(ren), as needed when my child(ren) are participating in outdoor activities during the summer months. I also know that the staff will follow re application to the areas of exposed skin.
Water Activities Participation Permission Form
YES, I give permission for my child to participate in any water activities
I understand that water activities include, water sprinklers, use of a pool, swimming in lakes and the ocean and boating. I also understand that injuries may occur from the result of participation in such water activities. I acknowledge and agree that my child may assume the risk of participating in such water activities and I understand that Let’s Create, LLC will not accept responsibility for any injuries sustained.
Summer Camp Enrollment Contract
- YES, I agree to this contract with Let’s Create, LLC to enroll the above child in this program for the dates listed.
I/We understand and agree that summer camp tuition payments are non-refundable. We understand that schedule changes will only be approved if there is enough space in the program for a different week of summer camp since space is limited, thank you for understanding. I/We understand and accept that Let’s Create, LLC will provide my child with responsible care, a safe and nurturing environment, and developmentally appropriate artistic instruction while in their care. Let’s Create, LLC has a “Zero Tolerance Policy” when it comes to abusive and/or aggressive behavior. Let’s Create, LLC reserves the right to dismiss a child, if it is in the best interest of the child and the program, for such behavior causes emotional and/or physical harm to oneself or others in the program. I/We agree to pick up our child from the studio no later than 5:30 pm, and agree to pay a late fee of $2 per minute beyond 5:30 pm. I/We agree to call the studio in advance if my/our child will be absent from the program for any reason, otherwise, I/We will receive a call from the studio to be sure my/our child is where they are supposed to be and safe. (Please note that attendance cancellations are not deducted from tuition. Thank you for understanding.) I/We agree and understand that tuition payments are to be made upon summer camp registration in the form of a check, cash, credit card, or money order. There is a $25 service charge fee per incident for nonsufficient funds. I/We agree to provide my/our child with a nutritionally healthy snacks, lunch, and beverages for them to eat at the studio each day of their attendance. The Studio will be open M-F 8:00 am-5:30 pm.
